PGI Membership FORMS for

New and Renewal Memberships

Welcome both fellow and future PGI Members!

This page is offered as a courtesy to anyone

who may in need of a renewal form.

For First-Time (New) Memberships, there is a SINGLE PAGE

Form to fill out and send in.

For Existing Members, there are actually two forms a

(Front and Back side) which need to be filled out.

Links to the forms are below.

Both forms are in your choice of Adobe PDF format or as JPG

(Picture) Files and may be downloaded, printed,

filled out and sent in with your payment.

If you are VERY familiar with Adobe and have the "FULL"

version of it, you can fill the form out using Adobe Acrobat and

then e-mail it to the PGI (address on form).

Some people may find it easier to download it as a JPG file,

fill it out using a "Paint" type program, Print it and send it that way.

Whichever works best for you.
